A Legal Disclaimer
The consultation services provided by Sarah DeLacey, Primary Care Provider, are intended solely for educational and informational purposes to support veterans navigating the VA disability claims process. I am not an attorney, accredited claims agent, or VA‑accredited representative, and I do not provide legal advice, legal representation, or direct claims filing services.
All recommendations, action plans, and Nexus Letters are based on my clinical judgment within the scope of my professional licensure in the State of Nevada. These materials are not guarantees of benefit approval and should not be interpreted as legal guidance.
Veterans are encouraged to seek assistance from a VA‑accredited representative, Veterans Service Organization (VSO), or legal professional for questions related to legal rights, appeals, or representation before the VA.
Use of these services does not create a provider‑patient relationship beyond the scope of the consultation, nor does it establish any legal or fiduciary relationship.
Billing Policy
I provide consultation services, record review, action plans, and Nexus Letters as a licensed medical provider in Nevada. These services require significant time, expertise, and clinical evaluation.
-
A fee schedule is published and available prior to booking.
-
Fees are billed upfront at the time of service.
-
A “pay‑what‑you‑can” option is available at checkout to ensure no veteran is turned away due to financial hardship.
-
Veterans who are unable to pay the full fee will still receive the same level of care, review, and documentation.
-
Optional Gratitude Contributions may be made after a veteran receives their VA award, but these contributions are voluntary and not required.
-
Payments are non‑refundable once services have been rendered, including record review or document preparation.
-
Nexus Letters and action plans will only be provided after the required clinical review is complete.
Privacy Policy
I am committed to protecting the privacy and confidentiality of all veterans who use my consultation services. All personal information, medical records, and communication are handled in accordance with applicable federal and state privacy laws, including HIPAA, where applicable.
